Fill the real state form from what's already in GHL
🔒 Internal staff tool — pulls the P2 clinical answers already on file and stamps them onto the actual DHS 1147 PDF. Not every P2 folder needs this — only run it when you're ready to submit.
1. Find the patient
Every patient in the pipeline who has clinical (P2) data started. Type any part of a name to narrow the list, then click it.

2. Generate
The list shows patients with an opportunity card in the pipeline whose P2 clinical folder has at least one field filled in — the ones actually worth generating a form for. Fills: patient name/DOB, the 11 scored sections (3–13) with the point value circled, computed Total Points (also saved back to the patient's record), primary/secondary diagnosis, assessed by/date, and functional-status notes. Sections with no answer yet in GHL are left blank — same as the paper form, finish those by hand. The file also saves to S3 automatically so there's always a record, even if you don't keep the download.
